How To Refer Patients To Oncology Nurse Navigators

If your patients and their families need help navigating through cancer treatment and survivorship, you can refer them to our oncology nurse navigators for much-needed support and resources.

Referral Through Epic

You can place an order for a referral to an oncology nurse navigator in Epic using completion matching, or it can be placed using the preference list. After the order is signed, it will route to the work queue and the patient will be contacted to schedule an appointment. An Epic tip sheet is available to assist with this process.

Non-Epic Referral

referral form is available to request a consult with an oncology nurse navigator. After filling out the form, send it via email to  [email protected] or fax it to (937) 223-9656.
