Cancer treatment can be complex and sometimes overwhelming for patients and their families. Premier Health’s oncology nurse navigators will use their in-depth knowledge of cancer care to guide patients through their journey and ensure their experience is as smooth as possible.
What Role Does a Nurse Navigator Play In Cancer Care?
A nurse navigator is an important point of contact during cancer treatment, helping with:
Education. A navigator may teach about:
- Phases of treatment
- What patients can expect
- Potential side effects
- Medications
Barriers. A navigator may connect patients with care partners to help eliminate barriers like:
- Difficult-to-schedule appointments
- Access to transportation and lodging
- Insurance and financial issues
Resources. A navigator may put patients in touch with hospital and community resources like:
- Dietitians
- Support groups
- Exercise programs
- Meals on Wheels
- In-home caregivers
- Equipment (walkers, wigs, etc.)
Are Premier Health Oncology Nurse Navigators Specially Trained?
Our oncology nurse navigators are knowledgeable about the best practices in cancer care, and they incorporate these as they work closely with patients and other cancer team members.
Many navigators have advanced degrees in nursing and specific certifications for oncology care.
How To Refer Patients To Oncology Nurse Navigators
If your patients and their families need help navigating through cancer treatment and survivorship, you can refer them to our oncology nurse navigators for much-needed support and resources.
Referral Through Epic
You can place an order for a referral to an oncology nurse navigator in Epic using completion matching, or it can be placed using the preference list. After the order is signed, it will route to the work queue and the patient will be contacted to schedule an appointment. An Epic tip sheet is available to assist with this process.
Non-Epic Referral
A referral form is available to request a consult with an oncology nurse navigator. After filling out the form, send it via email to oncologynavigator@premierhealth.com or fax it to (937) 223-9656.
