How To Refer Your Patient
Refer By Phone or Paper Form

- Rehabilitation and sports medicine: Call the rehabilitation location you want to refer your patient to for a referral form.

Transfer Your Patient
The Regional Referral Center is open 24 hours daily to coordinate the transfer of patients to a Premier Health facility. To arrange a patient transfer, call (800) 251-CARE (2273).
