Patient Remote Monitoring

Patient remote monitoring is a way for Fidelity Health Care nurses to closely monitor your wellness while you recover at home. Using tablets and Bluetooth-enabled devices provided and installed by Fidelity staff, we watch your progress and provide you with the education you need to recover and regain your independence. Through patient remote monitoring we can:

  • Monitor your weight, blood pressure, blood oxygen level, and other important measures of your health
  • Remind you to check your weight, blood pressure, etc., at a designated time each day
  • Know when your health is progressing as it should, and when there is a reason for concern. Our alert system will prompt us to contact you if your numbers (weight or blood pressure, for example) aren’t where they should be.
  • Enable you to easily send a message to your Fidelity caregiver if you have questions or concerns. You can also schedule a video call with your Fidelity caregiver to talk live from the tablet.
  • Enable a family member or friend to track your progress on our caregiver’s app
  • Help you avoid unwanted trips to your doctor’s office or the emergency department

What Equipment Will Fidelity Provide To You?

If you are eligible for patient remote monitoring, we will provide you the equipment you need, along with the education, to make patient remote monitoring easy for you. We understand some patients are not comfortable using computers and other technology that patient remote monitoring requires. But for patients who are able and willing, we will provide:

  • A tablet that will be used to record information and communicate with your Fidelity caregiver
  • Equipment that is needed (based on your particular health condition). This might include a scale, blood pressure monitor, pulse oximeter, or other devices.
  • Smartphone app to enable your designated family member to follow your progress
  • Set-up and education on how to use each device
  • Trouble-shooting if a device is not working properly

Most patient remote monitoring patients are those who have recently been discharged from the hospital and need monitoring to be sure they are recovering at home as expected.
