How We Protect Your Information
Patient health information is confidential. All records are protected by HIPAA. Additionally, medical records created under the federal 42 CFR part 2 program for treatment of substance abuse are protected in accordance with the law.
The medical information entered in our system is only accessible by providers and staff in Premier Health. This ensures that when you visit a Premier Health primary care office, specialist office, or hospital, that physician or advanced practice provider has your most accurate and up-to-date medical information.
This is especially important if you have a medical emergency or illness that requires hospitalization. Medical records cannot be released to any person without the permission of the patient or the patient's legally authorized representative (unless allowed by law). What your Medical Record Contains
- Your health history (what you tell the health care providers about your past and present health status)
- Your medical examination findings (what the health care providers observe when you are examined)
- Visit Documentation
Other information in your medical record may include:
- Lab test results
- Medications prescribed.
- Plans for further care, including instruction for self-care and return visits.
- Communication between your health care provider and any other health professional contributing to your care.
- Immunization records can be found in MyChart if you see a Premier Physician Network provider.
- Imaging test results
What Your Medical Record Does Not Contain
- Birth certificate: If you or your child was born at a Premier Health hospital, you will need to request a copy of your birth certificate from the appropriate birth county's office of vital statistics.
- Imaging Tests: Imaging test results such as X-ray, MRI, and nuclear medicine are part of your record. To request copies of the images, contact the medical imaging location where you had the test done. If you received the imaging test in an urgent care or a specialty practice office, contact the urgent care or practice.
