MyChart Proxy Access

MyChart® proxy access allows a user to access and manage another person's MyChart account. This feature is commonly used by parents of minor children and by spouses, caregivers, guardians, or those with power of attorney to assist in managing the care of a loved one.

Important notes/considerations:

  1. Due to the sensitive nature of medical information, each adult must create their own MyChart account.
  2. The proxy has full or partial access to the patient's medical information, so it's crucial to ensure trust and privacy are maintained. 
  3. The patient can revoke the proxy's access at any time.
  4. Proxy access is generally intended for family members, caregivers, or others who have a legitimate need to access the patient's information. 
  5. All proxy access will end when the primary account holder is marked deceased in the medical record.

Proxy Access for a Child

Parents/guardians are granted access to a child’s accounts until the child reaches the age of 12. There is no username or password for children under 12; it is linked to the parent/guardian’s MyChart account. After the 12th birthday, a child's account becomes their own and they may elect to share it with their parent or guardian at their discretion.

Proxy access can be set up by completing the MyChart Child Proxy Form (PDF).

Cancellation of Proxy Access for a Child 

Proxy access is automatically revoked when the patient turns 12.   

After 12 years of age, the patient has full control of their own MyChart account and can reinstate or change Proxy Access if they choose.  

Renewal of Proxy Access for a Child

Full proxy access remains in effect until the child’s 12th birthday.  After the 12th birthday, the child can elect to renew or edit proxy access.   

Proxy Access for an Adult 12+

Adult patients (12+) can grant proxy access to individuals they choose by appointing a proxy through their MyChart account or by completing the MyChart Adult Proxy Form (PDF).

Adult Categories

  • Full Access: Access to all clinical, messaging, and scheduling information.
  • Clinical Only: Only clinical information is viewable.  This setting restricts access to messages & scheduling.
  • Scheduling and Messaging Only: Only scheduling and messaging information is viewable.  This setting restricts access to clinical information.

Cancellation of Proxy Access for an Adult 12+

An adult who has voluntarily given their consent to allow proxy access may revoke the access immediately and at any time through MyChart account settings.

Adults may notify Medical Records/Health Information Management, in writing, of the desire to revoke proxy access. Allow for 3 business days for this change to be reflected on the chart.

Renewal of Proxy Access for an Adult 12+

Proxy access for adults expires annually on the date it was last elected.

Adult patients can renew or edit proxy access through:

  • Settings in MyChart (immediate)
  • A Premier Health office staff member (immediate)
  • The Medical Records/Health Information Management team (allow for 3 business days for this change to be reflected on the chart)

Guardianship Proxy Access

Under circumstances that compromise a patient’s judgment or ability to completely understand and make decisions about their health, a proxy guardianship authorization may be justified.  Such circumstances may include but are not limited to mental disability, dementia, or other health or mental issues.  The parent or guardian applying for guardianship proxy access is required to have legal guardianship or power of attorney for the individual for whom they are seeking proxy access, or the individual will need to sign an authorization allowing access.  

This access level is set by the office and not an available selection through MyChart. 

Revocation of Guardianship Proxy Access

Children or individuals with limited capacity must have legal documentation to establish the ability to revoke access.
