Lifestages Centers for Women is contracted with a variety of insurance providers to help patients be able to get the care they want and need. If you have any questions about our financial policies, please call our office at (937) 277-8988.

Insured Patients

Patients with insurance must bring their insurance card(s) to each appointment so our office can verify your benefits and financial responsibility with your insurance company. We will file a claim with your primary and secondary insurance companies. 

You are responsible for the payment of a co-pay and/or deductible at the time of service. If you have a large out-of-pocket payment for obstetric care, delivery or a surgery, however, we will create a payment plan to help manage the expense.

Self-Pay Patients

Patients without insurance coverage are expected to pay at the time of their visit or procedure. We provide our self-pay patients with an estimate of charges before their visit to help them plan ahead. We also work with patients to create a payment plan for obstetric care, delivery and surgeries.

Balance Due

Lifestages Centers for Women sends patients a monthly statement of any balance due. Our office team also will remind you of any balance due when you call to make an appointment and when you visit one of our offices.

We accept cash, checks, money orders, and credit cards – American Express, Discover, MasterCard, and VISA. After 30 days, any balance on your account is considered your responsibility, whether or not your insurance company has processed or paid your claim.

Patients will receive two statements and a final notice before their account is sent to a collection agency. If your account goes to collections, our practice will not be able to provide care for you until your account is paid in full.

Proudly Delivering at Miami Valley Hospital

At Lifestages Centers for Women, we proudly deliver at Miami Valley Hospital. Miami Valley Hospital (MVH) delivers more babies than any other hospital in the region. MVH has southwest Ohio's only high-risk maternity center and Level III Neonatal Intensive Care Unit in the same facility, which ensures mothers and babies receive the most comprehensive care.

Hospital Pre-Registration

We encourage all mothers-to-be to pre-register for their birthing experiences. Taking the time to complete paperwork before you arrive to deliver your baby will make your check-in quicker, allowing us to have you in your room in less time. Pre-registration can be completed at your hospital's registration office or online.
