Get Back to Your Life with Cardiac Rehab

With the help of Premier Health Cardiac Rehabilitation programs, more heart patients are returning to active lifestyles. The assistance of highly trained staff and the use of a variety of equipment ensures safe, steady progress as program participants work to improve their heart's health. Cardiac rehabilitation programs are divided into three phases.

Phase I

Cardiac rehabilitation begins while patients are in the hospital and recovering from their cardiac event or surgery. The goal of rehabilitation during this phase is to provide education and introduce Phase II.

Phase II

Once they've returned home and with their physicians' approval, patients can advance their recovery plan of care with Premier Health outpatient services. Phase II includes a detailed assessment, tailored exercise program with individual supervision, nutrition counseling, heart monitoring, and education classes.

Education classes may include:

  • Cardiac risk reduction strategies
  • Developing healthy eating habits
  • Reducing stress
  • Smoking cessation

Phase III or Maintenance

Heart patients who have graduated from Phase II and are committed to making cardiovascular fitness a lifelong pursuit, may choose to enroll in a maintenance program. During this phase, participants can continue their recovery progress in a familiar environment with group interactions and support, while they transition from monitored to independent exercise. Maintenance programs are paid out-of-pocket; payment arrangements vary by site.
