What to Expect

Upon arriving at your initial appointment, you will be asked to fill out a Patient Registration Form and a Medical History Form.

In addition, you may be asked to complete a Medical Records Release Form, which allows our office to obtain medical records from your previous doctors.

If you are a new patient, please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). Established patients should bring new insurance card(s) when applicable. You may be asked to complete a new Patient Registration Form during your first visit of each new year.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

After your visit, you will receive instructions for making a follow-up appointment. If needed, you will also receive information about prescriptions, blood work, or other orders.

You also will be given the opportunity to sign up for MyChart – an electronic medical record which allows you to review your medical information online and communicate with the team at Women’s Health Specialists & Midwives of Dayton through the secure Website. MyChart also allows you to schedule appointments, check your current and past lab results and ask other medical questions. View the MyChart Prescription Tip Sheet to see how to request prescription refills.

If tests have been ordered, you will receive a follow-up phone call or a letter with your results.
