Appointment/Procedure Preparation

To schedule an appointment, please call the office at (937) 208-6920(937) 208-6920 and choose the option for your physician.

You will be given specific instructions at the time your appointment is made, as preparation may vary by physician and type of visit. 

What to Expect

We ask new patients to arrive 15 minutes prior to their initial appointment to complete a Patient Registration Form. This form is available to download online if you would like to fill them out in advance. 

In addition, you may be asked to complete a Medical Records Release Form, which allows our office to obtain medical records from your previous doctors.

What to Bring

If you are a new patient, please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). Established patients should bring new insurance card(s) when applicable.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

After your visit, you will receive instructions for making a follow-up appointment for monitoring of chronic health conditions. Your “after-visit summary” will indicate when you should return, orders and/or instructions from your visit. If tests have been ordered, you will receive a follow-up phone call, a letter with your results or a MyChart email.
