Welcome to Premier Primary Care - Brookville. Our team looks forward to providing you with the care you need for yourself and your entire family. From routine checkups and preventive screenings to managing chronic conditions, we’re here to support your health every step of the way. We accept patients of all ages.

Schedule an Appointment

To schedule an appointment, please call our office (937) 833-4103(937) 833-4103. Any special instructions for your appointment will be discussed with you during the initial phone conversation. You can also schedule an appointment online or through MyChart.

For your convenience, we offer same-day care appointments. These appointments fill fast, so call us early in the morning, if possible. Our phone lines open at 8 a.m.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

If you are coming in for a blood draw, you may need to fast prior to your appointment. This will be discussed when you schedule your appointment.

Please bring a copy of your insurance card(s) and a list of your current medications to your appointment.

Your Appointment

Please bring your health insurance card(s) and a list of your current medications, including any vitamins, supplements, and over-the-counter medications, along with a list of allergies, to your appointment. If you are seeing other providers (specialists, etc.), bring their names and addresses, as well.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

When you are low on medication or have no refills, please schedule an appointment with a provider. Some patients may need to have lab work done.

If you are coming in for a blood draw, you may need to fast prior to your appointment. This will be discussed when you schedule.

If you are coming in for medication refills that require fasting labs, please plan to have your blood drawn at least two days before your appointment. This ensures that we will have the results at your appointment.

After Your Visit 

At the end of your visit, you will receive an After Visit Summary which will include your physician's instructions and appropriate education materials or suggestions.

If you don’t already have a MyChart account, you will be asked to sign up so that we will be able to communicate via our secure website as needed. Learn more about what MyChart features and how to create an account.

For chronic illnesses and medication follow-ups, appointments will be scheduled as needed. 

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network. Premier Primary Care - Brookville accepts most of these insurance plans.
