Patient Guide

Cardiothoracic Surgery Associates is accepting new patients. Our patients are generally referred to us; however, to schedule an appointment you may contact our office at (937) 208-3220(937) 208-3220. 

Due to the nature of our specialty, it may be necessary to change the date and time of your appointment should an emergency arise.

Office hours are 8 a.m. to 4:30 p.m. Monday through Friday.

Cardiothoracic Surgery Associates is dedicated to providing personalized, compassionate care. Our highly skilled surgeons and the highly trained team devote quality time to each of our patients so that we can truly understand their individual needs. We recognize the stress that is involved with cardiothoracic disease, and offer the highest level of care and comfort to our patients and their families.

Appointment/Procedure Preparation

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

Please be prepared to present the following at the time of your appointment:

• Insurance card(s)
• Photo ID
• Complete list of medications, including the dosage
• Name and address of your family physician
• New Patient Registration Packet

After your visit, your questions will be answered, and information will be provided regarding your next steps. This includes scheduling tests or surgery when needed.

Billing and Insurance
