Schedule an Appointment
To schedule an appointment, please call our office at (937) 297-8300(937) 297-8300. Any special instructions for your appointment will be discussed with you during the initial phone conversation.
For your convenience, we offer same-day care appointments. These appointments fill fast, so call us early in the morning, if possible. Our phone lines open at 8 a.m.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
- Patient Registration Form - Adult
- Patient Registration Form - Minor
- Patient History Form
- Medical Release Form
- Verbal Communications Form
What to Expect
Your Appointment
Please bring your health insurance card(s) and a list of your current medications, including any vitamins, supplements, and over-the-counter medications, along with a list of allergies, to your appointment. If you are seeing other providers (specialists, etc.), bring their names and addresses, as well.
All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.
When you are low on medication or have no refills, please schedule an appointment with a provider. Some patients may need to have lab work done.
After Your Visit
At the end of your visit, you will receive an After Visit Summary which will include your provider's instructions and appropriate education materials or suggestions.
If you don’t already have a MyChart account, you will be asked to sign up so that we will be able to communicate via our secure website as needed. MyChart also allows you to schedule appointments, check your current and past lab results, and ask other medical questions. Our office communicates lab and X-ray results promptly through MyChart. For chronic illnesses and medication follow-ups, appointments will be scheduled as needed.
Billing and Insurance
Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network. Premier Allergy Associates – Centerville accepts most of these insurance plans.
