We are committed to the timely distribution of COVID-19 vaccines to everyone in the community. Please answer the questions below to determine if you meet current eligibility requirements.

Are you trying to schedule a second-dose appointment?

By clicking yes below, you are agreeing to the following:

  • You received the Pfizer vaccine as your first dose
  • The appointment you schedule will be at least 21 days after you received your first dose
  • You will bring your vaccine card with you to your appointment (not required if your first dose was received at a Premier Health operated vaccine clinic)

Do you meet ALL of the above criteria?

Showing proof of vaccination at your vaccine appointment is required to ensure we are providing the right second dose (Pfizer only) at the right time.

Special Note: The Centers for Disease Control and Prevention (CDC) recommends that people with moderately to severely compromised immune systems receive a three-dose regimen of either Pfizer or Moderna vaccine to achieve the same level of immunity to two-dose vaccine series for people who are not immunocompromised.

  • I am 12 years of age or older AND
  • I received the Johnson & Johnson (Janssen) vaccine at least two months ago OR 
  • I received the second dose of the Pfizer or Moderna vaccine at least five months ago

Showing proof of vaccination at your vaccine appointment is required to ensure we are providing the right booster shot at the right time.

Special Note: If you are 12 through 17, you are only eligible for a Pfizer booster.

By clicking yes below, you are agreeing to the following in order to be eligible for a third dose:

You attest that your initial vaccine series (first and second dose) was either the Pfizer or Moderna vaccine, it has been at least 28 days since your last vaccine dose, and you meet one of the following criteria:

  • Individuals undergoing active treatment for cancer (solid tumor and hematologic malignancies). 
  • Individuals who have received a solid-organ transplant and are taking immunosuppressive therapy.
  • Individuals who have received a CAR-T-cell or hematopoietic stem cell transplant (within two years of transplant or taking immunosuppression therapy). 
  • Individuals with moderate or severe primary immunodeficiency (e.g., DiGeorge syndrome, Wiskott-Aldrich syndrome).
  • Individuals with advanced or untreated HIV infection. 
  • Individuals undergoing active treatment with high-dose corticosteroids (i.e., ≥20mg prednisone or equivalent per day), alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer chemotherapeutic agents classified as severely immunosuppressive, tumor-necrosis (TNF) blockers, and other biologic agents that are immunosuppressive or immunomodulatory.

Do you meet the above criteria to receive a third dose?

Showing proof of vaccination at your appointment is required to ensure we are providing the booster shot at the right time.

You are not currently eligible to receive your second-dose vaccine due to one of the following reasons:

  • We are only able to schedule second dose vaccination appointments for Pfizer at this time.
  • You cannot receive your second dose earlier than 21 days after your first dose.
  • Your vaccine card is important to prove the date and manufacturer of your first dose to ensure we are providing the right second dose at the right time.
