Appointment/Procedure Preparation

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

When you come for your appointment, please bring:

• A list of your current medications, including vitamins and herbal supplements
• Driver's license or photo ID
• Your health insurance card
• Completed paperwork if it was mailed to you in advance

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Premier Gynecologic Ongology accepts most of these insurance plans.
