Appointment/Procedure Preparation

Premier Health Primary Care - Brown Street is currently accepting new patients.  We strive to accommodate our patients needs with quality medicine and personalized care.

Please call (937) 208-9010(937) 208-9010 to schedule an appointment.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

Please bring your completed form, health insurance card, a list of allergies, and a list of medications you are currently taking to your appointment.

If you have a co-pay, please be prepared to make payment at the time of service.

After Your Visit or Procedure

After your visit to Premier Health Primary Care - Brown Street, we will give you instructions about a follow-up visit. We recommend that you schedule future appointments at this time.

You also will be given the opportunity to sign up for MyChart – an electronic medical record which allows you to review your medical information online and communicate with the team at Premier Health Primary Care - Brown Street through the secure Website.
