Miami Valley Infection Specialists wants to provide patients in the Dayton area with comprehensive, patient-focused care.

Our office is conveniently located at 30 E. Apple Street, Suite 6221. Ample parking is available in the parking garage. A fully enclosed walkway to the hospital entrance from the parking garage is accessible. 

We are currently accepting new patients and prefer to accept patients through primary care provider referral.

Appointment/Procedure Preparation

The physicians and team at the Miami Valley Infection Specialists are prepared to work with you in the treatment and care you need. To make an appointment, please call (937) 208-6630.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete a Patient Registration Form and a Medical Information Form.

In addition, you may be asked to complete a Medical Records Release Form, which allows our office to obtain medical records from your previous doctors.

All patients should be prepared to bring:

  • Your medications or a list of the medications you are currently taking
  • A list of your known allergies
  • Photo identification
  • Your health insurance card(s)
  • Vision insurance card(s) (if applicable)
  • Patient registration forms, if you filled them out in advance.

What to Expect

Current patients will be asked to complete a Patient Registration Form and re-sign a Notice of Privacy Practices once each year.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit/Procedure

After your visit, you will receive instructions about follow-up appointments needed, tests or consultations. After you see the doctor, a medical assistant will provide follow up instructions and any lab test form you will need.

The doctor frequently requests that patients have lab tests, such as medical imaging or blood tests, completed by their next appointment.

If you have lab work or X-rays done during your visit, a medical assistant will contact you with the results. If you do not receive the results within a week of your visit, please call our office at (937) 208-6630. Test results also can conveniently be accessed through MyChart.
