Lewisburg Family Practice is committed to providing quality, patient-centered care to the Preble County community. We offer extended weekday and weekend hours, as well as same-day appointments in an effort to best serve your needs.

Appointment/Procedure Preparation

To schedule an appointment, please call our office at (937) 962-2618(937) 962-2618. Any special instructions or information regarding your appointment will be discussed with you prior to your visit.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

Please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

At the end of your visit, you will receive an After Visit Summary Report detailing any follow-up instructions. Our friendly staff is happy to assist you in scheduling your next appointment. We will call you with any test results, or to follow up on test scheduling or referrals to specialists.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network physicians here. Lewisburg Family Practice accepts most of these insurance plans.
