Appointment/Procedure Preparation

To schedule an appointment with Hyatt Family Care, please call one of the following physician numbers:

Hyatt Family Care accepts most insurance carriers.

What to Expect

When you come to our office you will be asked to bring a photo ID and your health insurance information along with any co-payment that you are responsible for. We ask new patients to arrive 15 minutes prior to their initial appointment to complete a Patient Registration Form. This forms are available for download if you prefer to fill them out in advance.

In addition, you may be asked to complete a Medical Records Release Form, which allows us to obtain medical records from your previous doctors.

In order for our physicians to better serve you, we ask that you bring all prescription medications that you are currently taking to all of your office visits.  These prescriptions will be reviewed with you and all necessary refills will be given to you while you are in the office. We will also review our prescription refill policy at this time.

After Your Visit

At the end of your visit, if your diagnosis requires follow-up care, we have access to an organized network of specialty physicians within Premier Physician Network. 

In addition, if you need to be hospitalized, we will refer you to our hospital specialists who will care for you during your hospital stay. 

Hyatt Family Care will be happy to coordinate your follow-up care because we believe that prevention is the key to your overall health.
