Appointment/Procedure Preparation

Any special instructions regarding your appointment will be discussed with you during your initial phone conversation to make an appointment.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

Please be sure to bring your medications, a list of your allergies and your insurance card(s). All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

When your visit is done, you will be given an After Visit Summary, which includes the details of your visit, instructions, educational materials and suggestions. You also will be given the opportunity to sign up for MyChart – an online tool which allows you to review your medical information online and communicate with the team at Franklin Family Practice through the secure Website.

Billing & Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network online. Franklin Family Practice accepts most of these insurance plans.
