Appointment/Procedure Preparation

To schedule an appointment with any of our physicians, please contact our office at (937) 208-7600(937) 208-7600. We will provide any special instructions at the time of your call.

What to Expect

Prior to your appointment, you will be required to fill out a Patient Registration Form. You may also be requested to complete a Personal Health History Form. If you are unable to download the forms, we can mail them to you. Otherwise, we recommend you arrive approximately 15 minutes early to complete the necessary forms. All new patients are required to bring a picture ID on the first visit. Please bring your insurance card to each visit.

After Your Visit or Procedure

You will be contacted by phone or e-mail or check your MyChart messages within 10 days following completion of a lab or test. If the results are urgent we will contact you by phone.

Billing and Insurance

Beavercreek Family Physicians does not accept workers’ compensation cases.
