Updated Referral Guidance Following Premier ENT Associates Closure

Premier ENT Associates in Middletown will be permanently closing effective December 31, 2026. With this closure, Premier Physician Network will no longer provide general ENT services. Premier Health will continue to offer allergy services through Premier Allergy Associates in Centerville, and hearing-aid related audiology services through Fidelity Health Care. For other general ear, nose, and throat medical services, we are pleased to partner with Southwest Ohio ENT Specialists (SOENTS), whose practice is prepared to accept patients at the following locations:

Dr. Sameep Kadakia, MD, and Dr. Mohamedkazim Alwani, MD, will remain with Premier Physician Network, providing complex head and neck reconstruction services. They can be reached via an Epic referral – Premier ENT Head & Neck Oncology & Reconstructive Surgery.
