To support efficient scheduling and ensure patients are matched with the most appropriate specialty expertise, we’ve compiled this quick reference guide to commonly used rehabilitation orders and descriptors.

Physical Therapy (PT) Orders

General PT

  • CON30043 – Physical Therapy: Eval and Treat: Standard PT for musculoskeletal, functional mobility, or non‑protocol post‑injury needs
  • CON30720 – Post‑Op PT Eval and Treat: Protocol‑driven rehabilitation following surgery
  • CON30712 – Consult to Pre‑Optimization PT Evaluation: Pre‑operative conditioning, education, and risk reduction

Specialty PT

  • CON30383 – Vestibular Rehab: Dizziness, balance disorders, BPPV
  • CON30387 – Pelvic Floor Rehab: Incontinence, pelvic pain, pre/post‑partum care
  • CON30385 – Lymphedema Therapy (OT/PT): Cancer, vascular or trauma‑related edema
  • CON30743 – Oncology PT: Cancer‑related fatigue, deconditioning, functional decline
  • CON30742 – EDS PT: Hypermobility and connective tissue disorders
  • CON30741 – Acute Spine PT: Acute cervical, thoracic, or lumbar conditions
  • CON30680 – Running PT: Gait analysis and running‑related injury management
  • CON30386 – Wheelchair Clinic (OT/PT): Seating, mobility, and equipment evaluation

Sports PT & Sports/Human Performance

  • CON30739 – Acute Athlete PT: Recent sports injuries
  • CON30740 – Overhead Athlete PT: Shoulder/elbow conditions in throwers
  • CON30738 – Concussion PT: Vestibular, exertional, and return‑to‑play rehabilitation
    • If needing vision and cognitive treatment, please use CON30201 – Neuro Outpatient Therapy
  • CON30575 – Consult to Sports & Human Performance: CASH Based Program for post rehab and/or performance 

Occupational & Hand Therapy 

  • CON30044 – Occupational Therapy Eval and Treat: ADLs, driving screen, upper‑extremity function, cognition, vision, and return to work
  • CON30384 – Hand Therapy (OT/PT): Non‑operative upper‑extremity conditions
  • CON30716 – Post‑Op Hand Therapy (OT/PT): Surgical hand or upper‑extremity protocols

Speech Therapy & Neuro Services

  • CON30266 – Speech Therapy Eval and Treat: Communication, voice, swallowing
  • CON30746 – Speech Therapy Cognitive Assessment: 1x SP cognitive assessments comprised of testing and score review of the following:​
    • Montreal Cognitive Assessment (MOCA)​
    • Repeatable Battery for the Assessment of Neuropsychological Status (RBANS)​
    • When a full neuropsychological examination is NOT needed.
  • CON30698 – Speech Therapy: Head & Neck Oncology
  • CON30201 – Neuro Outpatient Therapy: Complex neurologic diagnoses that require multidisciplinary care or single service PT/OT/SP
