Schedule Your First-Dose COVID-19 Vaccination Appointment

 

We are committed to the timely distribution of COVID-19 vaccines to everyone in the community. Please answer the questions below to determine if you meet current eligibility requirements.

Are you or the person you are scheduling an appointment for:

  • Age 12 or older

Do you meet the above criteria?

If you are ages 12 to 17, your parent or guardian must come with you to your vaccination appointment.

Premier Health is only able to schedule vaccinations for people that meet ALL of the criteria below:

  • I have not received my first dose of COVID-19 vaccine already.
  • I have not received convalescent plasma or monoclonal antibodies in the past 90 days.

Do you meet ALL of the above criteria?

If you schedule a vaccination without meeting these criteria, your appointment will be cancelled.

Special Note: The Centers for Disease Control and Prevention (CDC) recommends that people with moderately to severely compromised immune systems receive a three-dose regimen of either Pfizer or Moderna vaccine to achieve the same level of immunity to two-dose vaccine series for people who are not immunocompromised. 

You are not currently eligible for a COVID-19 vaccination due to one of the following reasons:

  • If you are younger than age 12, there currently are no COVID-19 vaccines authorized for use in children of this age.
  • If you already received a COVID-19 vaccination, please sign up for a second-dose appointment. Please note: Premier Health locations currently are only offering Pfizer vaccine.
  • If you received convalescent plasma or monoclonal antibodies, it is recommended you wait 90 days before receiving a COVID-19 vaccination.

Currently there are no COVID-19 vaccines authorized for use in children younger than age 12.
