Appointment/Procedure Preparation

We are currently accepting new patients. To schedule an appointment, please contact us at (937) 372-7583(937) 372-7583. You may receive special instructions at the time of your call, depending on the nature of your visit.  We look forward to seeing you!

New Patients

If you are coming to us as a new patient, you will need to sign a Release of Information form for us to get your records sent to us from your previous primary care provider.  You can find this form here in advance of your first appointment with us.  We look forward to meeting you!

What to Expect

Please arrive 15 minutes before your appointment.  To ensure that we have the most current insurance and demographic information, you will be asked to verify your information and to produce your insurance card at each and every appointment.  Please note that a new Patient Information Form will need to be filled out any time there are any changes.  You can download this form here in advance of your appointment.

Please bring your insurance card(s), along with any medications you are currently taking, to each appointment.

After Your Visit or Procedure

If you have labs or other tests done during your visit, we will contact you within 3-4 days.  If you don’t hear from us within that time frame, please call our office.
