Appointment/Procedure Preparation

We are currently accepting new patients. To schedule an appointment, please contact us at (937) 372-7583(937) 372-7583. You may receive special instructions at the time of your call, depending on the nature of your visit.  We look forward to seeing you!

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

Please arrive 15 minutes before your appointment. To ensure that we have the most current insurance and demographic information, you will be asked to verify your information and to produce your insurance card at each and every appointment. Please note that a new Patient Information Form will need to be filled out any time there are any changes.

Please bring your insurance card(s), along with any medications you are currently taking, to each appointment.

After Your Visit or Procedure

If you have labs or other tests done during your visit, we will contact you within 3-4 days. If you don’t hear from us within that time frame, please call our office.
