Premier Hospitals Inspire Better Health with Updated Community Health Improvement Plan

As nonprofit hospitals, Premier Health facilities are required by the Internal Revenue Service (IRS) to conduct a Community Health Needs Assessment (CHNA) at least once every three years. This process, guided by the Affordable Care Act, ensures hospitals take a close look at the most pressing health needs in their communities and develop an implementation strategy to address them.

This spring, Premier Health’s hospitals completed updated Community Health Improvement Plans (CHIPs) for 2025–2027. These plans are based on data gathered through the regional CHNA, conducted in partnership with the Greater Dayton Area Hospital Association, local health departments, and community organizations.

The CHIP focuses on three key priorities:

  1. Barriers to Accessing Care – Increasing access and availability of care options, particularly for uninsured and underinsured community members.
  2. Elevating Delivery of Health Care Services – Expanding chronic disease management, prenatal care, and behavioral health interventions.
  3. Well-Being and Quality of Life – Promoting healthier lifestyles through education, prevention programs, and community partnerships.

We will advance this work through a robust network of programs, including mobile clinics, chronic disease prevention initiatives, prenatal care partnerships, substance use navigation, and health literacy campaigns. Programs like the Street Medicine Program, Barbershop Health Initiative, and Vaccines for Children mobile clinics will ensure care reaches those who need it most.
