Premier Health, June-July 2023

Premier Health Partners (“Premier Health”) is providing notice of a recent data security event that may have affected the personal information related to certain individuals.  At this time, there is no evidence of any identity theft or fraud occurring as the result of this incident. The confidentiality, privacy, and security of information is one of Premier Health’s highest priorities and Premier Health takes this matter very seriously.

On July 12, 2023, Premier Health became aware of suspicious activity related to certain systems within the Premier Health environment. Premier Health immediately launched an investigation to determine the nature and scope of the activity with the assistance of third-party forensic specialists and took steps to increase the security of our systems. Through the investigation, it was determined that files on certain Premier Health systems were subject to unauthorized access and acquisition by an unauthorized party at varying times between June 7, 2023 and July 12, 2023. As a result, Premier Health began reviewing the affected systems to determine what, if any, sensitive information was contained within the systems in question. While this review is still in process, on August 17, 2023, we determined that information related to certain individuals was impacted by this event. Once our review is complete, we will provide notice to any additional parties whose information is determined to be impacted by this event.

Premier Health has, however, determined that the following information may have been subject to unauthorized access for certain individuals during this incident: name and medical treatment and diagnosis information.  Once we determined whose information was impacted, we compiled address information for the impacted individuals, in order to notify these individuals.  Premier Health is providing notice of this incident to the individuals whose information we currently know is impacted.  Premier Health also notified required state regulators and the U.S. Department of Health and Human Services, Office for Civil Rights.

Premier Health encourages potentially impacted individuals to remain vigilant against incidents of identity theft and fraud, to review account statements, and to monitor their credit reports and explanation of benefits forms for suspicious activity. Premier Health is providing potentially impacted individuals with contact information for the three major credit reporting agencies, as well as providing advice on how to obtain free credit reports and how to place fraud alerts and security freezes on their credit files.  The relevant contact information is below:

Potentially impacted individuals may also find information regarding identity theft, fraud alerts, security freezes and the steps they may take to protect their information by contacting the credit bureaus, the Federal Trade Commission or their state Attorney General.
