When I arrived for my appointment, the receptionist was very courteous and professional. I had forms to fill out for updates. That went very well. My wait time was very short. The lady that took me in was very pleasant and she handled everything very professionally. After seeing my physician, I was sent to the lab. The Phlebotomist was very good. I had a wonderful experience throughout my visit.

I was uncomfortable signing the form about patient behavior. I don't know what happened to precipitate the form but it made me uncomfortable. Is this necessary??
