Breast Cancer Family History Assessment

Mammograms and breast exams are valuable tools for the early detection of breast cancer. However, some individuals could be at higher risk for breast and related cancers due to personal risk factors or family history and could benefit from additional tests.  The Breast Cancer Family History Assessment can help identify individuals who could have a higher risk for breast and related cancers, based on evidence-based guidelines from the National Comprehensive Cancer Network (NCCN). This is not a complete review of all breast cancer risks. For a complete evaluation of your risks, see your health care provider.

This assessment contains a maximum of 17 questions. The number of questions you will answer will vary depending on your responses.
