If you meet current eligibility requirements and want to schedule via phone, you can contact our call center between the hours of 8:30 a.m. to 5 p.m., seven days a week.

We are committed to the timely distribution of COVID-19 vaccines to everyone in the community who wants to be vaccinated. Please answer the questions below to determine if you meet current eligibility requirements set forth by the Ohio Department of Health.

Premier Health is only able to schedule vaccinations for people who meet certain criteria. Are you:

  • Age 16 or older

Premier Health is only able to schedule vaccinations for people that meet ALL of the criteria below:

  • I have not received my first dose of COVID-19 vaccine already.
  • I have not received any vaccine in the past 14 days.
  • I have not received convalescent plasma or monoclonal antibodies in the past 90 days.

If you schedule a vaccination without meeting these criteria, your appointment will be cancelled.

*Health insurance will not impact your ability to receive the COVID-19 vaccine. Vaccine doses will be given to Ohioans who choose to receive them at no out-of-pocket cost. Per government requirements, the administration fee will be covered by a patient’s public or private insurance company or, for uninsured patients, by the federal Health Resources & Services Administration’s Provider Relief Fund.

You are not currently eligible for a COVID-19 vaccination due to one of the following reasons:

  • If you are younger than age 16, there currently are no COVID-19 vaccines authorized for use in children of this age.
  • If you already received a COVID-19 vaccination, please return to the same vaccine provider for your second dose. This will ensure consistency in administration of the correct vaccine.
  • If you recently received a vaccination (other than COVID-19), please wait a minimum of 14 days before receiving a COVID-19 vaccine.
  • If you received convalescent plasma or monoclonal antibodies, it is recommended you wait 90 days before receiving a COVID-19 vaccination.

Whether or not you have health insurance, or what kind of insurance you have, does not impact your ability to receive the COVID-19 vaccine. However, this scheduling tool limits our ability to schedule your vaccine appointment online. We apologize for the inconvenience.
