Schedule Your COVID-19 Vaccination In Premier Health MyChart

When you’re eligible to receive the COVID-19 vaccine based on Ohio’s phased approach to vaccine administration, you will receive an appointment request in MyChart. Scheduling through MyChart is the most efficient way to sign up for your vaccination. If you don’t already have a Premier Health MyChart account, contact your provider’s office to request access, then activate your account.

Steps To Schedule In MyChart

1. Get Notified

When it’s time to schedule, you will receive an email from Premier Health MyChart with the subject line: New scheduling ticket available in Premier MyChart

2. Get Scheduled

  • Sign in to your Premier Health MyChart account through the Premier Health MyChart mobile app or website.
  • Click Make an Appointment next to the calendar icon.
  • You will be prompted to schedule an appointment for the COVID-19 vaccine. Select Schedule Now.
  • You will be prompted to answer a couple of questions. Do so and click Continue.
  • Choose your preferred location. 
  • Read the patient instructions specific to the vaccination.
  • Select the date and time for your appointment. Please note that vaccine supplies are limited, and appointments are subject to availability. 
  • Verify your information by clicking This information is correct.
  • Verify the appointment details, then click Confirm Appointment to finish.

3. Get Prepared

Once you’re scheduled, watch for an email from Premier Health MyChart with information to help you prepare for your appointment.

4. Get Your First Shot

Please arrive on time to keep the process running smoothly. After your shot, you will need to remain for 15 to 30 minutes for observation. We’ll also schedule the appointment for your second dose before you leave. 

5. Get Your Second Shot

Depending on the type of vaccine you receive, this will be 21 to 28 days after your first shot. Be sure to complete the eCheck-In process for this appointment as well.

Optional: Request MyChart Access For a Loved One

If you have a Premier Health MyChart account, you can request access to a loved one’s account to help them with scheduling and managing their care. 

To Sign Up For MyChart

Signing up for MyChart is easy:

  • On the MyChart activation page, provide all information requested.
  • If the information you enter matches our records, you will be prompted to create a username and password to complete the activation process.
    • MyChart username. This should be difficult for others to guess but easy for you to remember. Your user name is permanent: once created, it cannot be changed.
    • Password. This should be a unique combination of numbers (0-9) and letters, preferably using both uppercase and lowercase letters. Remember that your password will be case-sensitive. Your password must be different from your MyChart username.
    • Security question. This question will be used to verify your identity if you forget your MyChart password. Choose a security question from the list and enter your answer. Your answer cannot include your MyChart password.
  • On the next page, choose whether you want to receive a notification message in your personal email when there is new information available in your MyChart account. If you opt to receive email alerts, please enter your email address. To receive an alert when you’re eligible to schedule your COVID-19 vaccination, please opt to receive email alerts.
