Your health is important to us, and that's why we offer a broad scope of services to care for our patients at all stages of their lives. At Premier Women's Center, our patients are at the center of everything we do.
Appointment/Procedure Preparation
Premier Women's Center is currently accepting new patients. To schedule an appointment, please call (937) 335-1660(937) 335-1660. Any special instructions regarding your appointment will be discussed with you at the time of your call.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
Please be sure to bring either your medications or a list of the medications you are currently taking along with your identification and insurance card(s). All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.
We receive many requests for various forms for completion including FMLA forms. We are happy to help with these forms. Fax, mail or drop off the necessary forms after filling out your required sections of the document. Please allow 7-10 business days for us to sign and complete the paperwork.
There is a fee for completion of these forms.
Prescription Refills
If you need a prescription refill, please log into your MyChart account and submit a request through the prescription refill option. You may also call the office. Please allow 1-2 business days for us to process the request. There are special circumstances under which our physicians may want to see you before approving a refill request.
All medication refill requests must be received in the office no later than 2:00 pm on Thursday to be refilled before the weekend. No new medications or medication refills will be done over the weekend.
After Your Visit or Procedure
After your visit, you will receive follow-up instructions. You will also receive information about your procedure, prescriptions, blood work or other orders/testing if needed. If tests have been ordered, you will receive a phone call or letter to notify you of your results.
You will also be given the opportunity to sign up for MyChart so we can communicate via our secure website as needed. MyChart also allows you to schedule appointments, check your current and past lab results, and ask other medical questions.
Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network. Premier Women's Center accepts most of these insurance plans.
Hospital Pre-Registration
We encourage all mothers-to-be to pre-register for their birthing experiences. Taking the time to complete paperwork before you arrive to deliver your baby will make your check-in quicker, allowing us to have you in your room in less time.
