Appointment/Procedure Preparation

What to Expect

We ask that you arrive 15 minutes before your appointment time. We may need to verify that the information we have is correct, so please have your identification and insurance card ready.

Piqua Family Practice wants to make your office visit as comfortable and efficient for you as possible. To help shorten your waiting room time, download and complete your patient registration form at your convenience and bring it with you on your first visit.

You may be asked to review and sign additional forms when you arrive for your visit.

After Your Visit or Procedure

We strive to process laboratory test information efficiently, contacting you within three business days whenever possible. We make our best effort to provide a 24-48 hour turnaround time to your pharmacy for prescriptions and refills. The turnaround time over holidays and weekends may be from 48 to 72 hours.

Billing and Insurance
