At Middletown Infectious Disease Associates, we work hard to provide you personalized, compassionate care in a welcoming environment.

Appointment/Procedure Preparation

To schedule an appointment, call our office at (513) 974-4071(513) 974-4071. We ask new patients to arrive 15 minutes prior to their initial appointment to complete a Patient Registration Form.

In addition, you may be asked to complete a Medical Records Release Form, which allows our office to obtain medical records from your previous doctors.

All patients should be prepared to bring:

  • Your medications or a list of the medications you are currently taking
  • A list of your known allergies
  • Photo identification
  • Your health insurance card(s)
  • Vision insurance card(s) (if applicable)
  • Patient registration forms, if you filled them out in advance

After Your Visit or Procedure

At the end of your visit you will receive an After Visit Summary which will include your physician's instructions and appropriate education materials or suggestions. You will be asked to sign up for MyChart so that we will be able to communicate via our secured website, back and forth, as needed.

MyChart also allows you to schedule appointments, check your current and past lab results, and ask other medical questions.  Our office communicates lab and X-ray results promptly either by phone, letter, or MyChart.

For chronic illnesses, follow-up appointments will be scheduled every three months.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Middletown Infectious Disease Associates accepts most of these insurance plans.

Established patients are encouraged to sign up for MyChart, a confidential web-based messaging system for convenient communication with your health care provider.
