Survivorship Support

Survivorship begins with your patients’ cancer diagnosis and continues as they live beyond cancer. Premier Health’s cancer team creates a personalized survivorship care plan with a treatment record, potential short-term and long-term effects, guidelines for follow-up and future testing, and symptoms to report to the doctor.

More than 10 million people in the U.S. today are cancer survivors. Three out of every four American families will have at least one family member diagnosed with cancer.

Cancer survivorship includes the physical, emotional, and day-to-day issues of the cancer journey. Survivorship begins at the time of diagnosis and continues during and after treatment.

Survivorship Care Plans

Once you have completed treatment, your survivorship needs include follow-up health care and getting help with any continuing emotional and physical effects of cancer or treatment. You will work with your doctor or other members of your medical team on creating a Survivorship Care Plan.

A survivorship care plan helps to ensure the best possible follow-up care and quality of life. 

Your personalized plan is a record of your cancer and treatment history. It will tell you what checkups or follow-up tests you may need in the future. In general, it may include the following:

  • Your diagnosis
  • Stage of cancer
  • Hormone level, which can depend on the type of your cancer
  • Molecular markers, which can depend on the type of your cancer
  • Treatment start and end dates
  • Contact information for all members of your cancer team
  • Cancer tests and other exams
  • Short-term or long-term effects from your cancer treatment
  • Symptoms you should report to your doctor
  • Recommendations from your doctor for future cancer screening, prevention, and healthy living choices.  

Cancer Information Center 

Premier Health’s Cancer Information Center, commonly known as the Registry, is a data system for collection, management, and analysis of information on all cancer patients diagnosed and treated at our facilities.

The Registry is maintained for both educational and research purposes, as well as for lifetime patient follow-up. It also serves as a reminder to you and your doctors to schedule regular examinations so any recurrence of the disease can be detected early.

Tree of Life

The Tree of Life represents a celebration of hope, courage, and life shown through photographs of survivors, caregivers, and those who treat cancer patients.
