Discretionary Funds

The programs we consider for discretionary support will impact the health of individuals located in our 8 county service area; Montgomery, Miami, Greene, Darke, Shelby, Clark, Warren, Butler counties.

How to Apply for Funds

  • Submit Letter of Intent (LOI) through our online system.  The LOI provides Premier Health with an overview of your funding request.  
  • After your LOI has been reviewed and determined that your project falls within our funding guidelines, you will download, fill out and submit the full grant application.
  • Only one LOI per organization per grant cycle may be submitted for consideration.
  • Organizations that are funded are eligible to apply again after the grant funds have been paid and the final report has been submitted and approved. If an organization’s final report is past due and no attempt has been made by the organization to extend the final report deadline, the organization will become ineligible to apply for a grant for a period of two years.
  • If an LOI is denied, the organization may not apply for the same project again.
  • Typically, the Committee awards funds for no more than 25 percent of a total project budget, not to exceed $75,000; however, it reserves the right to increase or decrease the amount awarded if circumstances warrant.  

2018 Deadline Dates

All LOI’s and Grant Applications are due by 4:00 pm on the Deadline Date

  • Letter of Intent: January 15, 2018
  • Contribution Application: March 2, 2018
  • Contribution Awards Announced: March 29, 2018
  • Letter of Intent: May 4, 2018
  • Contribution Application: September 1, 2018
  • Contribution Awards Announcement: October 5, 2018
