Community Health Improvement
Building healthier communities is a systemic change we have embraced for a long time at Premier Health and is part of our mission. We are committed to and are excited to support these initiatives that will positively affect so many of the people we serve. Every three years, nonprofit hospitals complete a Community Health Needs Assessment (CHNA), as required by law.

2013 Assessment and Improvement Plans
An essential part of knowing how we can improve health in our community is to understand the unique health issues of our community. To that end, Premier Health was part of a collaboration in 2013 with the Greater Dayton Area Hospital Association and hospitals throughout Southwest Ohio to conduct a regional Community Health Needs Assessment. This assessment assisted us in identifying areas of opportunity to improve community health.
This report shares our plan for improving population health in the identified priority areas in our region.
