Premier Health Patient Education

The Premier Health patient education committee supports the organization by optimizing the utilization of patient education tools and resources, thereby reducing variation, fragmentation, duplication, and costs. Patient education strategy supports achievement of Meaningful Use Stage 2 and provides clear, evidence-based information for patients and families.

Premier Health has purchased Krames StayWell for all patient reference material. Krames StayWell provides patient education and discharge instructions, which that can be accessed via “clinical references” in Epic. Drs. Reilling, McCorkle, and Petry are active committee members, and have provided insight and overview to the selection and implementation of Krames StayWell, as embedded into Epic, as well as related processes associated with a system approach to eatient education.

  • HSD departments converted from the discharge writer (DCW) to after visit summary (AVS), with clinical references.
    • ?All departments and areas were asked to review Krames patient education health sheets, in order to assure that content is valid with Premier Health procedures
  • Premier Health purchased Lexicomp as a clinician medication resource; Micromedex is no longer used.
    • Clinicians are able to access Lexicomp via link in Epic, both on toolbar and via the Premier Health intranet.

Premier inpatient after visit summary (AVS) has clinical references available within Epic, in order to ensure that the patient is receiving all relevant patient educational information related to both his/her problem list and medication list. The update is to assist with meeting some of the requirements for Meaningful Use.
