Hospital Survey on Patient Safety Culture

Your opinion matters. In 2012, the Agency for Healthcare Research and Quality (AHRQ) survey was completed across Premier Health, to assess the patient safety culture from a hospital staff perspective. Based on the results of the survey, the following processes were implemented:

  • Daily safety check-in (DCI), to increase safety awareness and the rapidity with which we respond to safety events
  • Improved communication during patient transfers
  • Incorporated the concept of “just culture” into HR policies
  • Quarterly patient experience booster training, including communication tools 
  • A standardized pre-operative and surgery checklist to improve OR communication

To get a pulse on additional opportunities to improve patient safety culture, Premier Health inpatient staff will be asked to complete the AHRQ Safety Survey from September 15-26, 2014. Patient safety culture is a critical component of health care quality. As we strive to improve, there is much emphasis on establishing a strong safety culture for the benefit patients, staff, and visitors. Your feedback is vital, relevant, and encouraged in order for all of us to improve safety at Premier Health.
