Community Health Improvement Plan

Building healthier communities is a systemic change we have embraced for a long time at Premier Health and is part of our mission. We are committed to and are excited to support these initiatives that will positively impact so many of the people we serve.

2015 Community Health Needs Assessment

What are the most important health or health-related issues affecting your community? Your opinion matters – it is important for us to hear what you and others in your area think. The feedback and ideas we receive will help shape future priorities. Premier Health is hosting a variety of community forums to solicit input. But if you are unable to attend a form, the survey is available onlineOff Site Icon.

2013 Assessment and Improvement Plans

An essential part of knowing how we can improve health in our community is to understand the unique health issues of our community. To that end, Premier Health was part of a collaboration in 2013 with the Greater Dayton Area Hospital Association and hospitals throughout Southwestern Ohio to conduct a regional Community Health Needs Assessment. This assessment assisted us in identifying areas of opportunity to improve community health.

This report shares our plan for improving population health in the identified priority areas in our region.

Community Health Needs Assessments

Every three years, nonprofit hospitals complete a Community Health Needs Assessment (CHNA), as required by law. Premier Health needs your input to help us build healthy communities.
